The following corporations are contracted to provide services/programs funded by the MJAAA: The Mifflin-Juniata Food Service, Elder Community Services, Call-A-Ride Service, and the Regional Services Corporation. These following contracted services are provided: Senior Center Services, Socialization, Recreation, Health Promotion, Volunteering, Congregate and Home Delivered Meals by Elder Community Services. Regional Service Corporation provides Ombudsman, Apprise, Information and Referral Services. Transportation is provided by Call-A-Ride Services. * Counseling Service is provided toconsumers who need professional therapeutic counseling. 0 consumers received this service.
* The MJAAA contracts for Personal Emergency Response System,who provided service in 69 homes. These systems provide a 24 hour service that allows persons living alone to get help just by the push of a button.
* Home Health Service is a consumer-choice program, funded by the MJAAA. The consumer can select a provider from a list of organizations to provide health aide services or nursing services. MJAAA is payer of last resort. No services used.
*Legal advice and representation primarily for the purpose of guardianship or protective services. 6individuals received 25.75 hours of service.
* 39 persons were assisted with their bathing, dressing, eating, grooming, etc. through the Personal Care Program.
*Home Support Service was provided for 39 individuals which provides basic management in their home and respite care. This ensures a safe and sanitary environment or helps to complete minor home repair.
* Home Modifications were made in 5 Waiverconsumer’s homes. These modifications help the older adult to remain safe and healthy in their homes. This service helps to assist with labor cost for home repairs.
* The Personal Assistance Service helped 47 individuals with daily hands-on care, respite care, and with home support activities.

* Assessment and Care Management The MJAAA completes assessments and level of care assessments for persons who might be in need of services as well as for persons seeking placement in a nursing facility, personal care home or domiciliary care home. A Pre-Admission Assessment is required for persons age 18 and older to determine the level of care that is required in compliance with Medical Assistance regulations. An Obra Assessment is completed for persons who are diagnosed with mental illness, mental retardation or other developmental disability and who seek placement in a nursing facility. For older adults who are determined to need services funded through the AAA, Care Management is provided for the development of a care plan, support in selecting serviceproviders, issuing service orders, monitoring the services and ongoing assessment of need. Staff completed 445level of care assessments; 279PA Screens (for services such as personal emergency response and home delivered meals); 312Care Management Instruments(CMI) to indentify met and unmet needs for persons to receive personal care and other services requiring care management; 110annual recertification's were done.
*Domiciliary Care: 9persons no longer able to live independently resided in a Dom Care home, which provides a protective home-like atmosphere in private homes throughout the area.
*Through the Family Caregiver Support Program,34 consumers received reimbursement to cover the care giving expenses related to paying for help or supplies for their loved one. State and Federal funding used.
* The Federal Funded Grandparenting Program, provided help (up to $100/month) to 2consumers, over 60 years of age, in order to provide care to a relative under the age of 18.
*96reports of need were made to Protective Services for investigation of suspected abuse, neglect or exploitation. The MJAAA helped these consumers with services to provide a safe environment for the older person. The Elder-Abuse/Neglect Hotline is a 24 hour service. Call 717-242-9454 or call collect if necessary.
*Nursing Home Transition12 consumers were successfully helped to move from a nursing home back to a home in the community. This service may assist with finding housing, set-up of a household, financial assistance with security deposits and purchases of household items and moving expenses.
* The Aging Waiver Programallowed 106 consumers to receive home and community based long term care services. These services are funded through a special waiver of Medicaid restrictions, it allows monies typically used for nursing home care to be used for their care in their home.
*The MJAAA was Guardianfor 14clients who are no longer able to manage their finances, make health care or personal decisions on their own. The MJAAA maybe appointed to act on behalf of the client when no other family member can act as guardian.

Thank you for your help and care with my sister-in-law. I have tried for years to get her to agree to accept services, that I felt would help. It seemed to take a crisis for her to agree. We all want to make our own decisions, and I respect that. I will try to be available when needed, as I have been the past 4 years, since moving back from Florida. I want to commend you for your care and your work with my sister-in-law. She has the potential to have a much better life because of you.
